Click on the check boxes to mark or unmark them. Print the form and mail it in! 

To clear the form, just click on the red box "Clear Form". The Clear Form box does not appear on your printed copy! You can still print the forms if you do not wish to fill them out on your computer.  The yellow areas do not appear on the printed copy, just the information you type in them will print.

    Please note: If you intend to apply for an IRS federal tax exemption as a charitable organization, your articles of incorporation must contain a required purpose clause and a dissolution of assets provision. Information on 501(c)(3) qualification is available on the IRS website, www.irs.gov. Click on the "Charities and Nonprofits" link and then the "Life Cycle of a Public Charity Link."
